State DOI / NAIC Regulator

Complaints against insurers for auto, home, property/casualty, life, and health policies: claim delays and denials, cancellations, and policy disputes.

Insurance is regulated at the state level. NAIC's consumer page points you to your own state DOI, which handles the actual complaint. Phone varies by state.

Complaints and grievances about Medicare, Medicare Advantage, and Part D plans: coverage, quality of care, plan service, and enrollment issues.

1-800-633-4227 = 1-800-MEDICARE, available 24/7. Unresolved issues can be escalated to the Medicare Beneficiary Ombudsman.
HealthCare.gov Regulator

Health Insurance Marketplace (HealthCare.gov)

Issues and appeals for federal Marketplace (ACA) health plans: eligibility determinations, enrollment problems, and coverage denials.

Marketplace Call Center (1-800-318-2596) is 24/7. Submitted appeals are handled by the Marketplace Appeals Center at 1-855-231-1751.

Deceptive or unfair business practices, billing problems, scams, false advertising, and general consumer fraud across any industry.

1-877-382-4357 = 1-877-FTC-HELP. Spanish site at reportefraude.ftc.gov. Reports feed law enforcement but the FTC does not resolve individual disputes.

State Attorney General - Consumer Protection

Consumer fraud, unfair or deceptive practices, and billing disputes under each state's consumer protection laws. The right office depends on your state.
